Filing Requirements for Dane County Board of Supervisors


Candidates for County Board Supervisor must meet the following eligibility requirements.

  • 18 Years of Age
  • Elector/Resident of Dane County


  1. Completion of Campaign Registration Statement – Form CF-1

    The GAB-1 form must be filed in the County Clerk’s Office prior to announcing your candidacy for office, expending/receiving funds for candidacy or circulating nomination papers. Continuing candidates must file an amended GAB-1 indicating the office sought and the new primary and election dates. The form must be signed by both the candidate and the treasurer if one has been appointed.

  2. Completion of Declaration of Candidacy – Form EL-162

    The GAB-162 form must include the candidate’s name as it would appear on the official ballot. A candidate may use their full legal name or any combination of first name, middle name, initials, or a nickname with their last name. No abbreviations or titles are allowed.

    A nickname is defined as a familiar or shortened form of a proper name by which an individual is commonly known. Names that aren’t familiar or shortened forms of proper names, such as “Skip,” “Red” or “Lower Taxes” are not allowed. A nickname is a substitute for the candidate’s legal name. It is not permissible to add the nickname in quotes between the first and last name.

  3. Completion of Nomination Papers – EL-169
    Collect 50 (minimum) to 200 (maximum) signatures

    Signatures of qualified electors (at least 18 years old and a U.S. citizen) may only be collected between December 1, 2019 and January 7, 2020. The deadline for filing the papers is January 7 at 5:00 p.m. Only electors who live in the supervisory district are allowed to sign the nomination papers.

    Signers must give their complete address (house number, street and municipality – no Post Office boxes), and must list the municipality of residence (city, village or township). Signers must date their signature, including the year.

    The circulator does not have to reside in the supervisory district. Circulators are only required to be qualified electors. The circulator must certify the nomination papers after obtaining the signatures. Signatures dated after the date listed by the circulator will not be counted. Signatures with addresses not in your district will not be counted. You may determine whether an address is in your district by checking

  4. Completion of Statement of Economic Interest

    Must be filed within 2 weeks of the filing deadline (January 21, 2020). Section VIII should contain your signature which must be witnessed (not notarized), the date you signed, your legibly printed name, and title (County Board Supervisor Candidate District #). Please read the instruction sheet for help in filling out the form.


The deadline for filing above forms is January 7, 2020 at 5:00 p.m.

Candidates for multi-jurisdictional municipal courts serving municipalities in multiple counties, please verify which county is the filing officer for the office.
